甲状旁腺腺瘤合并甲状旁腺功能亢进的临床分析

摘要:

目的 探讨甲状旁腺腺瘤合并甲状旁腺功能亢进的临床表现、诊治方法。方法 分析同济大学附属东方医院近年收治的2例甲状旁腺腺瘤合并甲状旁腺功能亢进症患者的临床资料。结果 患者均表现为不同程度的骨病变及血钙明显增高。手术患者均经超声、CT定位,手术方式为甲状旁腺腺瘤切除术,手术全部成功,手术后所有症状均有不同程度的缓解,化验指标恢复正常,无喉返神经损伤等并发症发生。结论 对甲状旁腺良性肿瘤合并甲状旁腺功能亢进症患者早期诊断,早期进行有效的病灶切除治疗,可减少严重骨病变的发生,减少致残率。

Abstract:

Objective To review the diagnosis and treatment of parathyroid adenoma complicated with primary hyperparathyroidism.Methods The clinical data of 2 cases of parathyroid adenoma complicated with primary hyperparathyroidism in East Hospital,Tongji University were retrospectively reviewed. Results The diagnosis of 2 patients was confirmed by operation and pathology.There were bone pathologic changes of varying extent and increased serum levels of calcium.Ultrasonography and CT scan were performed to obtain accurate diagnosis and lesion localization before operation.Minimally invasive parathyroidectomy was successfully completed and the primary operation cure rate was 100%.At postoperative the symptoms and signs were all relieved to varying extents,the laboratory results returned to normal,and there was no permanent hypoparathyroidism or recurrent nerve injury and other complications. Conclusion Early diagnosis and early surgical treatment for parathyroid adenoma complicated with primary hyperparathyroidism can prevent severe bone changes and deformity effectively.
