摘要:

目的 探讨P16^ink4α在宫颈上皮内瘤样病变发生发展的诊断价值。方法 分别对182例宫颈各类病变采用免疫细胞学方法检测P16^ink4α表达情况,并行人乳头状瘤病毒分型检测。结果 P16^ink4α阳性表达率为35.71%。P16^ink4α的表达与病理诊断分级呈正相关.宫颈良性病变、CIN Ⅰ、CINⅡ、CIN Ⅲ、宫颈癌病变表达率为3.77%、15.71%、87.50%、89.66%、100%。结论 P16^ink4α可作为宫颈良性病变与CIN Ⅰ以及CINⅠ与CINⅡ、CINⅢ和宫颈癌鉴别的重要标记物,且诊断特异性高于HPV。

Abstract:

Objective To investigate the expression of P16^ink4α in cervical intraepithelial neoplasia(CIN) and its clinical significance.Methods One hundred and eighty two biopsy specimens from cervical lesions were collected including 53 cases of cervititis,70 cases of CIN Ⅰ,24 cases of CIN Ⅱ,29 cased of CIN Ⅲ and 6 cases of squamous cell cancer.The expression of P16^ink4αwas detected by immunohistochemical method,and HPV DNA was detection by fluorescence-PCR assay.Results The overall positive rate of P16^ink4α in 182 cervical lesions was 35.71%.The P16^ink4α positive rates for cervical inflammaton,CIN Ⅰ,CIN Ⅱ,CIN Ⅲ and cervical cancer were 3.77%,15.71%,87.50%,89.66%,and 100%.There were significant differences in P16^ink4α rate between lesions≤CIN Ⅰand ≥CIN Ⅱ(χ²=12.71,P＜0.01).Conclusion P16^ink4α can be used as a marker to identify cervical cancer and advanced CIN,and its specificity is higher than that of HPV detection.
