肥胖对慢性肾小球肾炎患者氧化应激系统的影响

摘要:
目的探讨慢性肾小球肾炎慢性肾功能不全(CKD2-4期)患者肥胖与氧化应激系统的关系。方法选择慢性肾小球肾炎CKD2-4期的患者43例及正常对照组40例,分别按体质量指数(body mass index,BMI)分为正常组(BMI<24 kg/m2)、超重组(24≤BMI<28 kg/m2)和肥胖组(BMI≥28 kg/m2),应用酶联免疫吸附(ELISA)法检测血清中脂质过氧化产物前列腺素F2α水平(8-isoprostane F 2α,PGF2α)及用分光光度法测定血清中蛋白的晚期氧化蛋白产物(advanced oxidation protein products,AOPP)及抗氧化应激指标维生素E(Vitamin E,VitE)的含量。结果慢性肾小球肾炎超重组患者体内AOPP及PGF2α含量明显高于体质量正常组(P<0.05),肥胖组体内AOPP及PGF2α含量高于超重组,但差异无统计学意义(P<0.05)。超重组患者体内VitE含量明显低于体质量正常组(P<0.05),肥胖组患者体内VitE含量低于超重组,但差异无统计学意义(P>0.05)。相同BMI指数分组的慢性肾小球肾炎患者体内AOPP及PGF2α含量明显高于健康对照组(P<0.05),VitE含量明显低于健康对照组(P<0.05)。结论慢性肾小球肾炎患者体内存在氧化应激状态,且随着BMI指数升高,慢性肾小球肾炎患者体内氧化应激水平升高,抗氧化应激能力下降,提示肥胖通过氧化应激途径影响慢性肾小球肾炎患者。

Abstract:
Objective To investigate the relationship between obesity and oxidative stress in patients of glomerulonephritis with chronic renal insufficiency. Methods Forty three patients with primary chronic glomerulonephritis (PCGN) CKD-2 stage 2-4 and 36 healthy subjects (controls) were enrolled in the study. Each group was divided into three subgroups according to body mass index (BMI): normal weight group ( BMI<24 kg/m2 ), overweight group ( 24≤ BMI<28 kg/m2) and obesity group (BMI ≥ 28 kg/m2). The levels of 8-isoprostane F 2or (PGF2a) in serum was measured by enzyme linked immunosorbent assay (ELISA), advanced oxidation protein products (AOPP) and vitamin E (VitE) in serum were measured by spectrophotometry. Results The serum levels ofPGF2oL and AOPP in PCGN patients were significantly higher than those of healthy controls, and the serum level of VitE in PCGN patients was lower than this of healthy control (P<0.05 ). The serum PGF2ot and AOPP levels in overweight group of PCGN patients were higher than those in normal weight group, and serum VitE level was lower than that in normal weight (P<0.05 ). Conclusion The results suggest that the oxidative stress is present in patients with glomerulonephritis and the severity is correlated with the degree of obesity.
