腹腔镜下阑尾切除术疗效分析

摘要:

目的总结腹腔镜阑尾切除术的治疗经验，探索其开展的临床意义。方法回顾性分析腹腔镜阑尾切除术62例临床资料，其中慢性阑尾炎8例，急性单纯性阑尾炎1例，急性化脓性阑尾炎52例，急性坏疽或穿孔性阑尾炎9例。结果完成腹腔镜阑尾切除术56例，中转开腹6例。腹腔镜阑尾切除术平均手术时间40min，平均术后住院时间3．3d。术后并发症主要有：穿刺孔感染2例，腹腔脓肿2例，无腹腔出血，无肠管损伤、阑尾残端瘘。所有患者均康复出院。术后随访3～6个月无粘连性肠梗阻发生。结论腹腔镜阑尾切除术安全、有效，能明显减轻患者术后疼痛，缩短康复时间、降低切口感染率，其安全性及有效性均较高，值得临床推广。

Abstract:

Objective To evaluate the efficacy of laparoscopic appendectomy. Methods Data of 62 cases of laparoscopic appendectomy were retrospective analyzed, including 8 cases of chronic appendicitis, 1 case of acute simple appendicitis, 52 cases of acute suppurative appendicitis and 9 cases of acute gangrene or perforated appendicitis. Results Fifty-six cases had completed laparoscopic appendectomy, while 6 cases transferred to laparotomy. Average operation time was 40 min, average postoperative hospital stay was 3.3 d. The main postoperative complications were puncture hole infection in 2 cases and abdominal abscess in 2 cases; there no abdominal bleeding, bowel injury or appendix stump fistula. All patients were discharged with recovery, there was no postoperative adhesive intestinal obstruction in the follow-up of 3 -6 months. Conclusion Laparoscopic appendectomy is safe, effective procedure, it can significantly reduce postoperative pain, shorten healing time and decrease incision infection rate.
