目的 探讨胆囊组织中泡沫细胞形成与胆固醇结石和胆囊息肉发生的相关性。方法 收集胆囊样本58例,其中胆囊胆固醇结石34例、胆囊胆固醇息肉16例及对照组8例,制作石蜡切片,经HE染色及免疫组化方法检测胆囊黏膜下层泡沫细胞形成及CD14表达情况,分析与临床表现的相关性。结果 胆囊黏膜皱褶在胆囊结石组和胆囊息肉组中明显增多,其黏膜下层泡沫细胞检出率分别为50.0%和62.5%,均显著高出对照组12.5%(P<0.05),同时免疫组化结果显示这些泡沫细胞高表达CD14,提示这些泡沫细胞来源与巨噬细胞密切相关。结论 胆囊黏膜层内泡沫细胞形成和脂盾在黏膜内沉积与胆囊胆固醇结石和胆囊胆固醇息肉发病密切相关。 |

Abstract: |
Objective To investigate relations between formation of foam cells in the gallbladder mucosal layer and cholesterol gallstone or cholecystic polypus. Methods Fifty eight gall bladder samples were collected from cholecystectomy, including 34 samples with cholesterol gallstone, 16 with cholecystic polypus and 8 control samples. All samples were processed in paraffin section and stained by Hematoxylin-eosin. CD 14 expression was detected by immunohistochemistxy. The correlation between formation of foam cells in the gallbladder mucosal layer and the clinical features was analyzed by SPSS 17.0 statistical software. Results The gallbladder mucosal folds were marked increased in cholesterol gallstone and cholecystic polypus groups compared to controls. The positive rate of foam cells in the gallbladder mucosal layer from patients with cholesterol gallstone (50.0% ) and cholecystic polypus (62.5% ) were significantly higher than that in controls (12.5% , P<0.05). Furthermore,these foam cells expressed CD14 molecule which indicated that CD14 positive foam cells were derived largely from macrophages. Conclusion The results indicated that the presence of foam cells in the gallbladder mucosal layer may be a significant step in the pathogenesis of cholesterol gallstone and cholecystic polypus. |
