生物反馈治疗盆底功能障碍型便秘的疗效分析

摘要:

目的探讨生物反馈治疗对（盆底功能）障碍型便秘患者疗效。方法选择我院消化内科门诊就诊、符合罗马Ⅲ标准的73例功能性便秘患者,行肛门直肠测压对患者进行分型,对（盆底功能）障碍型便秘患者行生物反馈治疗及治疗后行肛门直肠测压、临床评估和随访。最后行统计学分析。结果73例患者行肛门直肠测压,其中24例患者为非（盆底功能）障碍型便秘,49例患者为（盆底功能）障碍型便秘,两组间直肠静息压比较10.33±7.69、14.82±9.75 mmHg,有显著差异（P＜0.05）。24例（盆底功能）障碍型便秘患者治疗后直肠静息压10.12±6.7 mmHg较治疗前15.23±7.21 mmHg显著降低（P＜0.05）,其中20例患者矛盾运动完全消失,患者治疗前后便秘症状总有效率达91.67%,伴随症状积分治疗前后比较总有效率达91.67%。结论生物反馈治疗（盆底功能）障碍型便秘疗效显著。

Abstract:

Objective To evaluate the biofeedback therapy in treatment of pelvic floor dysfunction constipation.Methods Seventy three outpatients with functional constipation meeting Rome Ⅲclassification were enrolled.The type of functional constipation was classified by anorectal manometry.The patients classified as pelvic floor dysfunction constipation underwent biofeedback therapy.At the end of treatment,the patients received anorectal manometry examination,clinical assessment and follow-up.Results Among 73 patients 49 were classified as pelvic floor dysfunction constipation and24 were non-pelvic floor dysfunction constipation.The resting rectal pressure of pelvic floor dysfunction patients was higher than that of non-pelvic dysfunction patients（14.82 ± 9.75 vs 10.33 ±7.69 mmHg,P<0.05）.Twenty four patients with pelvic floor dysfunction constipation received biofeedback therapy,after treatment,the resting rectal pressure decreased significantly（15.23±7.21 vs 10.12±6.75 mmHg,P<0.05）,and the paradoxical movement disappeared in 20 patients.The effective rate for constipation symptoms was 91.67% and the effective rate for symptom score improvement was 91.67% of biofeedback therapy.Conclusion Biofeedback therapy is effective in treatment of pelvic floor dysfunction constipation.
