摘要:

目的探讨应用Y-型BD密闭式留置针穿刺负压外引流治疗顽固性头皮积液效果和应用前景。方法同济大学附属同济医院神经外科自2009年2月至2013年3月共治疗9例顽固性头皮下积液患者,对7例钛板颅骨成形术后、1例脑外伤术后、1例胶质瘤切除＋去骨瓣减压术后继发顽固性皮下积液患者应用Y-型BD密闭式留置针穿刺负压外引流治疗,根据引流情况调节负压（维持负压约3-5 kPa）,保持引流通畅并防止管道缩瘪。每日记录引流量并观察头部积液情况,3 d内复查头颅CT,了解颅内术区情况,平均治疗时间3-6 d。结果 9例患者均恢复满意,局部体检和复查头颅CT均证实皮下积液完全消失,无感染或其他并发症发生。结论应用Y-型BD密闭式留置针穿刺负压外引流治疗顽固性头皮积液具有见效快、无痛苦、无创、无感染、操作简便等优点,及时规范、正确应用该技术可获得满意效果,值得临床进一步推广应用。

Abstract:

Objective To evaluate the efficacy of vacuum drainage with Y-shaped BD close type indwelling needle in treatment of intractable effusion under the scalp.Methods Nine patients with intractable effusion under the scalp were treated with vacuum drainage by using Y-shaped BD close type indwelling needle from February 1999 to Match 2013,including 7 cases with cranioplasty,1 case with brain trauma after operation and 1 case after surgery for glioma.The vacuum pressure was adjusted appropriately and the drainage was kept for 3-6 d.Results All patients recovered completely from the effusion,as confirmed by the CT scan the physical examination.No infection and recurrence occurred.Conclusion The vacuum drainage by BD needle is effective,safe and easy-to-apply for intractable effusion under the scalp.
