目的分析银屑病患者临床及实验室检查的特点和伴发疾病情况,揭示银屑病的某些发病规律与代谢性疾病之间的关系。方法 对上海市皮肤病医院2007年至2012年间380例住院银屑病患者进行回顾性临床分析,包括年龄、性别、体重、银屑病类型、发病年龄、病程、家族史、发病季节、加重诱因、合并症、既往特殊用药史、糖脂代谢等实验室检查等。结果 银屑病多见于男性,有银屑病家族史患者发病年龄较早,脓疱型银屑病较寻常型银屑病发病年龄早。银屑病发病诱因感染最常见,发病季节以冬季为主。银屑病合并症主要有:高血压(12.36%)、糖尿病(5.26%)、高脂血症(14.21%)、高尿酸血症(10.48%)。降血糖药物可能对银屑病有治疗作用。代谢性疾病相关各项实验室检查异常在各类型银屑病之间无显著差异,但高脂血症与银屑病伴发高血糖及肝功能异常有关。结论 银屑病患者易并发多种系统疾病,尤其是代谢相关性疾病。这些疾病与银屑病可能具有共同的遗传学、病理生理学基础。降糖药物的应用可能为银屑病的治疗开辟一条新途径。 |

Objective To analyze the clinical manifestations and laboratory findings of patients with psoriasis. Methods The clinical data of 380 patients admitted in Shanghai Dermatology Hospital from 2007 to 2012 were retrospectively analyzed. ResultsPsoriasis was more common in men than in women; patients with a family history were likely to have an onset at earlier age; the psoriasis pustulosa had an earlier age of onset than psoriasis vulgaris; the disease was more common in winter, and the infection was the most important etiology of the disease. Psoriasis was complicated with metabolic syndrome, including hyperlipidemia (14.21%), hypertension (12.36%), hyperlithemia (10.48%) and diabetes (5.26%). There were no significant differences in laboratory findings among different types of psoriasis, but the hyperlipidemia was closely associated with high blood sugar and liver function abnormalities in patients with psoriasis. Conclusion Psoriasis is complicated with a variety of diseases of metabolic syndrome, indicating that there might be a common genetic and pathophysiological basis and hypoglycemic drugs may be a new approach for treatment of psoriasis. |
