目的观察二氧化碳(CO2)激光灸与温针灸治疗轻中度膝骨关节炎的临床疗效差别,探讨其作用机制及安全性。方法选择轻中度膝骨关节炎患者80例,按照随机对照的原则分为CO2激光灸组和温针灸组,分别予CO2激光灸、温针灸治疗,4周为1个疗程。观察患者36条目简明健康调查量表、WOMAC量表得分及TNF-α、IL-6含量变化,对患者生活质量和关节整体功能进行评价。结果(1) CO2激光灸组在身体功能、身体疼痛、身体功能对角色的限制、关节疼痛、关节僵硬、日常活动难度得分均优于治疗前(P<0.05);但不及温针灸组。(2) 治疗4周时,CO2激光灸组TNF-α、IL-6均较治疗前升高,较治疗2周时下降,含量高于温针灸组。结论CO2激光灸通过调节血清TNF-α、IL-6等血清炎性因子从而改善膝骨关节炎患者身体功能、身体疼痛、身体功能对角色的限制、关节疼痛、关节僵硬,日常活动难度,远期疗效略低于温针灸。 |

ObjectiveTo the efficacy between CO2 laser moxibustion and warm acupuncture in treatment of mild to moderate knee osteoarthritis. MethodsEighty patients with mild to moderate knee osteoarthritis were randomly divided into CO2 laser moxibustion group and warm acupuncture group with 40 cases in each group, patients received corresponding treatment for 4 weeks. The FS-36 scale, WOMAC scale and the joint function were evaluated, serum TNF-α and IL-6 levels were measured and compared between two groups. ResultsThe scores of body function, body pain, limitation of body function to role, joint pain, joint stiffness and difficulty of daily activities in CO2 laser moxibustion group were significantly improved compared to those before treatment(P<0.05); however, the extent of improvement was less than that of warm acupuncture group. After 4 weeks of treatment, serum TNF-α and IL-6 levels in CO2 laser moxibustion group were increased compared to those before treatment, but lower than those after 2 weeks of treatment; the levels of above indicators were higher than those of warm acupuncture group. ConclusionCO2 laser moxibustion can effectively improve the body function and relieve the symptoms in patients with knee osteoarthritis by adjusting serum inflammatory factors. The long-term efficacy is slightly lower than that of warm acupuncture. |
