OSAHS-COPD重叠综合征患者血清脂联素和瘦素的表达及临床意义

Abstract:
目的 探讨阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome, OSAHS)合并慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)(OSAHS-COPD重叠综合征,简称OS)患者血清脂联素和瘦素的表达情况以及与病情的相关性。 方法 选择2022年1月—2024年1月就诊的稳定期COPD患者120例作为研究对象,其中30例为COPD组,另外90例合并有OSAHS的患者为OS组。根据睡眠呼吸暂停低通气指数(apnea hypopnea index, AHI)的结果,OS组又分为3个亚组: 轻度OS组、中度OS组、重度OS组,每组30例。统计各组患者的年龄、性别、体质量指数(body mass index, BMI)等基本情况,检测各组患者的血清脂联素和瘦素的表达水平,分析这些指标与OS严重程度、BMI之间的关系。 结果 与COPD组相比,OS各亚组患者的BMI和血清瘦素水平均显著升高(P<0.05),而血清脂联素水平则显著下降(P<0.05)。血清脂联素水平与OS患者AHI呈负相关(P<0.05),血清瘦素的表达水平与OS患者AHI呈正相关(P<0.05)。 结论 OS患者的血清脂联素表达水平与病情的严重程度呈负相关,而血清瘦素表达水平与病情的严重程度呈正相关。血清脂联素和瘦素有望作为评估OS患者病情严重程度的有效指标之一。

Expression and clinical significance of serum adiponectin and leptin in patients with OSAHS-COPD overlap syndrome

Abstract:
Objective To investigate the expression of serum adiponectin and leptin in patients with both obstructive sleep apnea hypopnea syndrome and chronic obstructive pulmonary disease(OSAHS-COPD overlap syndrome, OS) and its correlation with the state of illness. Methods A total of 120 stable COPD patients in Tinglin Hospital from January 2022 to January 2024 were enrolled as research subjects;among them, 30 were categorized as the COPD group, while the remaining 90 COPD patients, who were complicated with OSAHS, were set as the OS group. Based on the sleep apnea-hypopnea index(AHI) results, the OS group was further classified into three subgroups: mild OS, moderate OS and severe OS, with 30 patients in each subgroup. The data of age, gender, and body mass index(BMI) were recorded in the groups. The serum adiponectin and leptin levels were measured, and their relationship with OS severity, BMI, adiponectin, and leptin were analyzed. Results Compared with the COPD group, the BMI and the serum leptin levels in all OS groups were significantly increased(all P<0.05), while the serum adiponectin levels were significantly decreased(all P<0.05). In OS patients, the serum adiponectin level was negatively correlated with AHI(P<0.05), while the level of serum leptin was positively correlated with AHI(P<0.05). Conclusion Serum adiponectin level is inversely correlated with OS severity, whereas serum leptin level exhibits a positive correlation with OS severity. Serum adiponectin and leptin have the potential to serve as effective indicators for evaluating the severity of OS.
