目的 通过功能磁共振成像鉴定骨关节炎的不同阶段并探讨趋化因子CCL2/CCL3及受体CCR5的表达。方法 选取因骨关节炎(osteoarthritis, OA)行人工全膝关节置换术(total knee arthroplasty, TKA)患者的离体股骨髁软骨标本,进行超短回波磁共振成像(ultrashort echo magnetic resonance imaging, UTE MRI)扫描。标本扫描结束后行Safanin-O染色及Mankin评分,根据Mankin评分结果将标本分为2级OA 组(Mankin评分2~5分)、3级OA组(Mankin评分6~9分)、4级OA组(Mankin评分10~14分);每组6例,共18例患者。免疫组织化学法检测18例患者的离体股骨髁软骨标本CCL2、CCL3、CCR5的表达。结果 相对于Mankin评分低的2级OA组,Mankin评分较高的3级OA组及4级OA组的患者膝关节功能磁共振成像提示骨关节破坏严重;相对于2级OA组,3级OA组及4级OA组的患者CCL2、CCL3、CCR5的表达增加;相对于3级OA组,4级OA组的患者CCL2、CCL3、CCR5的表达增加。结论 根据功能磁共振成像显示骨关节炎的严重程度可推测CCL2、CCL3及CCR5的表达。 |

Objective To explore the expression of CCL2/CCL3 and CCR5 in different stages of osteoarthritis identified by functional MRI(fMRI). Methods The isolated femoral condyle specimens of patients undergoing total knee arthroplasty(TKA) due to osteoarthritis(OA) were collected. The specimens underwent ultra-short to echo-time(UTE) MRI scanning, then the Safanin-O staining was performed and Mankin score was assessed. According to the Mankin scores, the specimens were divided into level 2 OA group(Mankin score 2-5 points), level 3 OA group(Mankin score 6-9 points), and level 4 OA group(Mankin score 10-14 points). There were 6 specimens in each group and total 18 isolated femoral condyle specimens were subjected to immunohistochemical staining, and the expression levels of CCL2, CCL3, and CCR5 were evaluated. Results Compared with level 2 OA group the fMRI of level 3 OA group and level 4 OA group showed severe bone and joint destruction; the expression of CCL2, CCL3, and CCR5 in level 3 OA group and level 4 OA group increased. Compared with level 3 OA group, the expression of CCL2, CCL3, and CCR5 increased in level 4 OA group. Conclusion The results of this study suggest that it is possible to infer the expression of CCL2, CCL3 and CCR5 based on the severity of osteoarthritis identified by functional MRI. |
